We get it: There’s no place like home. That’s why it can be so difficult when the time comes to contemplate how best to provide care for our loved ones who can’t quite manage on their own. 

 

At St. Tammany Health System, we strive to make that decision a little easier with our team of Home Health caregivers, all of whom are skilled, highly trained and, above all, compassionate.  

 

On top of all that, they provide that care conveniently in the comfort of our patients’ owns homes.

Services

Skilled nursing

Skilled nursing offers a variety of services including IV therapy, wound care therapy, post-operative care, remote patient monitoring for chronic conditions with nurse oversight daily, lymphedema therapy, and Big/Loud program for Parkinson’s patients provided on an intermittent basis for prescribed medical treatment, supportive care, patient and primary caregiver teaching.

Social services

Board-certified social workers aid patients and families with financial assistance, community services and/or long-term care placement. They help patients adjust to changes resulting from illness or injury.

Nutritional guidance

The nurse teaches the patient and family special diet instructions in the home and follows up with the patient to discuss how the diet is going.

Home health aides

Trained and experienced nursing aides assist with personal hygiene and activities of daily living under a registered nurse’s direction.

Physical, occupational and speech therapy

A licensed therapist designs and provides rehabilitative services for each individual patient in the home. Specialized therapeutic exercises help patients meet their maximum rehabilitative potential.

The St. Tammany Health System Hospice program provides supportive care to terminally ill patients and their families in the home through an interdisciplinary team that addresses physical, psychological, social and spiritual needs.
