That’s why our Home Care Services programs have become such an integral and appreciated part of our community. Here, our patients aren’t just customers. They’re members of our extended family, and we treat them that way.
Providing rehabilitative and supportive care to patients confined to their homes and under their physician’s care, our professional and medically trained team members are committed to providing quality care – with an emphasis on the word “care.”
At St. Tammany Health System, our job doesn’t end at the hospital doors, thanks to our Home Care team, members of which still make house calls – and who prove St. Tammany Health System is much more than a hospital.
All of our Home Care Services programs are not the same. Here’s a brief primer:
Provides an annual checkup with one of our nurse practioners at no out-of-pocket cost to Medicare Part B participants. Taking place at your doctor’s office or your home, on this visit the provider will develop an individualized care plan just for you.
A specialized care team follows patients in their own home to ensure improvement in conditions, such as congestive heart failure and chronic obstructive pulmonary disease by preventing hospital readmissions with tools including remote patient monitoring.
This department of Home Care Services provides nursing and rehabilitation therapy in the home setting. While allowing patient to continue to enjoy the familiar surroundings of their own home.
Devoted to the improvement of quality of life for people living with a chronic serious illness by addressing the many stressors that can accompany such illness, including symptom management, difficulty coping, and documentation of healthcare wishes.
This department of Home Care Services provides supportive care to terminally ill patients and their families in the home through an interdisciplinary team that addresses physical, psychological, social and spiritual needs.
