Now’s your chance to help make St. Tammany Parish a little healthier, and in just 10 minutes.

The Metropolitan Hospital Council of New Orleans has launched its 2021 Community Health Needs Assessment, a public survey designed to gauge community health needs throughout the region.

The 10-minute survey is conducted every three years to address population health and barriers that prevent St. Tammany Parish and other communities across the state from being as healthy as they can be.

The survey, available in English, Spanish and Vietnamese versions, can be accessed through the following links:

Feel free to share it with family, friends and anyone else with an interest in improving healthcare metrics in Louisiana.

The 2021 Community Health Needs Assessment is conducted in collaboration with the Metropolitan Hospital Council of New Orleans (MHCNO) and its 19 partner hospital facilities*. We are also working in collaboration with the Louisiana Department of Health as they conduct their State Health Assessment.

The survey shared here is the same survey currently being shared by the 19 MHCNO partner hospital facilities and the Louisiana Department of Health. If you have already received and completed this survey from one of our partner organizations, please do not take this survey again.
