Birth Certificates
St. Tammany Health System’s Health Information Management Department assists parents with applying for their newborn’s birth certificate.
After your baby’s birth, you will receive a birth certificate packet to complete while in the hospital. Beginning at 32 weeks gestation, you will also have the option to complete the birth certificate packet and submit it prior to your child’s birth.
Upon receiving your completed packet, a birth certificate clerk will contact you and use the information provided to apply for your baby’s birth certificate. Parents will receive a complementary birth verification letter to use for verification of birth until they receive a certified copy of their baby’s birth certificate from the Louisiana Office of Vital Records (usually 3-6 months after birth).

Parents will receive a birth verification letter after completion of their baby’s birth certificate packet. The purpose of this document is to confirm that a birth certificate has been applied for by the hospital. Parents may need this form to submit to their health insurance or for their baby’s first pediatrician visit. To obtain an additional birth verification letter, please contact the STHS Birth Certificate Office.
The Louisiana Office of Vital Records provides parents with a certified copy of their baby’s birth certificate within 3-6 months after birth. If you need to obtain an additional birth certificate, please contact Vital Records at (504) 593-5100. Please note that there is a charge to receive an additional copy of a Louisiana birth certificate.
If you need to make a correction to your baby’s birth certificate (i.e. misspelled name, incorrect date of birth, etc.), contact the STHS Birth Certificate Office within 3 months of your baby’s birth. Any corrections required later than 3 months after birth will need to be submitted to the Louisiana Office of Vital Records. You can contact their office at (504) 593-5100.
Parents will be given the option to request a Social Security Card for their baby when completing their birth certificate packet. It typically takes 2-4 months to receive your baby’s Social Security Card. If you need one sooner, you may apply in person at any Social Security Administration office.
