STQN Announces Fourth Quarter 2017 Medical Director’s Award
COVINGTON – St. Tammany Quality Network (STQN) presented its 2017 fourth quarter Medical Director’s Award to Andrew Baier MD for his efforts associated with reducing hospital readmissions, which improves patient outcomes and satisfaction. Dr. Baier, STPH hospitalist, is a member of the readmissions committee and physician coach working to maximize outpatient resources for improved patient recovery.
“Andrew has been an integral part of our readmissions committee,” said Michael K. Hill MD, STQN medical director and governing board chair. “His work on the committee and with his fellow physicians has resulted in improved patient care and reduced readmissions.”
Dr. Baier is a founding member of the readmissions committee and has been on the STPH medical staff for five years. As a practicing hospitalist, he brings a unique perspective to patient needs upon discharge and is a patient advocate. He has been instrumental in making significant changes to the discharge process, which has improved physician engagement and understanding of the components of a successful discharge. Specifically, he highlighted the need for physicians to consider the patient’s social needs as an important aspect of discharge planning.
