A look at the three types of annual wellness visits STHS's Primary Wellness Home Program can offer
This free, “once in a lifetime” benefit is for patients who are within the first 12 months of enrolling in Medicare Part B. During this in-home visit, the provider will record your height, weight, blood pressure, body mass index and vision. Among other things, the provider will also go over your medical history and provide you with information and referrals specific to your personal healthcare needs.
Patients who have been enrolled in Medicare longer than a year will skip the initial preventative physical exam and instead receive their first annual in-home wellness visit, in which many of the same basic health assessments are performed. That includes, among other things, a review of your family and medical history, a screening to detect cognitive issues, general health advice, as well as referrals as needed.
Once you’ve had your initial visit – whether it’s the initial preventative physical exam or an initial annual wellness visit – you qualify for an annual in-home wellness visit. During this visit, your care provider will record your height, weight, blood pressure, body mass index and vision, in addition to reviewing your medical history, performing a health risk assessment, screening for cognitive issues, and providing health advice and referrals as needed.
