Primary Care

WHEREVER YOU GO, THERE WE ARE.

With primary care clinics in Covington, Folsom, Mandeville and Madisonville, our St. Tammany Physicians Network offices are ready to deliver world-class health care close to home – wherever your home happens to be.

St. Tammany Health System

Founded in 1954 as St. Tammany Parish Hospital, St. Tammany Health System has grown over the decades to include numerous primary care clinics throughout western St. Tammany Parish, from our on-site clinic at St. Anthony's Garden assisted living facility to our Express Care walk-in clinic to our St. Tammany Physicians Network clinics, which meet the outpatient needs of the community from newborn to geriatric care in Covington, Mandeville, Folsom and Madisonville.

All of our primary care clinics partner with Care Harmony to serve eligible patients who have Humana Managed Medicare or traditional Medicare. They offer chronic care management assistance for patients living with more than one chronic condition - such as arthritis, diabetes, depression or high blood pressure - and provide monthly telephone check-ins to help with your care between doctor's visits.

Whether it’s for acute care, chronic illnesses or an annual physical for your job, school or sports, our board-certified physicians or nurses are on call 24/7 and have convenient access to the wealth of resources St. Tammany Health System has to offer, including:

    • Diabetes education
    • Radiology services
    • Respiratory services
    • Physical therapy
    • Occupational therapy
    • Speech therapy
    • Our Sleep Disorders Center

Primary care your way

Because healthcare isn’t a one-size-fits-all thing.

Traditional clinic visits

Traditional clinic visits

Whether you’ve already got a doctor or you need a new one, our St. Tammany Physicians Network clinics offer traditional primary care visits for the whole family in four convenient locations throughout western St. Tammany Parish:

  • Covington
  • Folsom
  • Mandeville
  • Madisonville
  • Express Care

    Express Care

Not everybody works a regular 9-to-5, which can make getting routine healthcare difficult. With that in mind, St. Tammany Health System operates its Express Care walk-in clinic, which provides all the services you’d expect from a traditional medical office but in non-traditional hours.

    Primary wellness home care

    Primary wellness home care

In addition to helping detect disease early, when treatment is more effective, in-home wellness visits can also provide those who have chronic conditions with resources to help manage their condition. The best part: It’s all covered by Medicare Part B, so it comes with no additional expense for qualifying patients.
